Dental practice receipt template

The same exempt-from-tax, adjustment-ladder structure as a medical practice, with a treatment-plan balance that patients pay down over several visits.

Free, no signup, no watermark. PDF, PNG and JPG export at the format’s real paper width.

The lines a dental practice actually bills

These are the preset line items this page loads into the maker. The price bands are typical, not prescriptive — set your own. The tax column is guidance: materials are taxable in most states and service labour often is not, which is a real per-state distinction covered on the state requirements pages.

The format this trade issues

A patient payment receipt with a per-visit ledger and a running treatment-plan balance, and no tax line.

Get Receipt renders it as Medical co-pay receipt A5 clinic receipt: patient and visit block, the co-pay amount collected at the desk, and a note that the balance is billed to the insurer.

The full field order, alignment grid and physical spec for that format is documented on the medical payment receipt spec page.

Common questions

How do I receipt a payment against a treatment plan?
Show the plan total, the amount received today, and the balance remaining. Without the balance line the patient has no way to know how much of the plan is left, and neither does your front desk.
Is dental treatment taxable?
No — dental services are exempt from sales tax. Retail items you sell alongside them, such as a whitening kit, generally are not exempt.
What does an insurance adjustment line mean?
It is the difference between your charge and the contracted allowed amount. Printing it explicitly is what makes the patient-responsibility figure below it verifiable rather than arbitrary.

Card details on this receipt

Get Receipt prints at most four card digits and has no expiry field anywhere in the app. FACTA forbids printing more than the last five digits, or the expiration date at all, on a cardholder receipt — so a violating receipt is not something this tool can be made to produce, by hand or by the assistant.
