Medical practice receipt template

Medical services are almost universally exempt from sales tax, so the format has no tax line at all — it has an insurance adjustment waterfall instead. FSA and HSA claims need the service description, not just the amount.

Free, no signup, no watermark. PDF, PNG and JPG export at the format’s real paper width.

The lines a medical practice actually bills

These are the preset line items this page loads into the maker. The price bands are typical, not prescriptive — set your own. The tax column is guidance: materials are taxable in most states and service labour often is not, which is a real per-state distinction covered on the state requirements pages.

The format this trade issues

A patient payment receipt with the charges, adjustment, insurance-paid and patient-responsibility ladder — and no tax line, because there is no tax.

Get Receipt renders it as Medical co-pay receipt A5 clinic receipt: patient and visit block, the co-pay amount collected at the desk, and a note that the balance is billed to the insurer.

The full field order, alignment grid and physical spec for that format is documented on the medical payment receipt spec page.

One thing this trade has to carry

A patient payment receipt normally carries the provider’s name and national provider identifier.

Common questions

Why is there no tax line on a medical receipt?
Because medical services are exempt from sales tax essentially everywhere. The insurance adjustment ladder — total charges, adjustment, insurance paid, patient responsibility, amount paid today, balance — occupies the space a tax line would.
What does an FSA or HSA claim need?
The provider, the date of service, a description of the service, and the amount the patient actually paid. An explanation of benefits is not a receipt, and a card statement is not either.
Should procedure codes print?
A procedure code and a plain-language description together. The code is what a plan administrator matches; the description is what the patient can read.

Card details on this receipt

Get Receipt prints at most four card digits and has no expiry field anywhere in the app. FACTA forbids printing more than the last five digits, or the expiration date at all, on a cardholder receipt — so a violating receipt is not something this tool can be made to produce, by hand or by the assistant.
