Restaurant receipt template
The guest check and the card slip are two different documents with two different jobs. The check itemises and suggests a tip; the slip authorises the card and carries the write-in tip line.

The lines a restaurant actually bills
These are the preset line items this page loads into the maker. The price bands are typical, not prescriptive — set your own. The tax column is guidance: materials are taxable in most states and service labour often is not, which is a real per-state distinction covered on the state requirements pages.
| Line item | Billed by | Typical | Tax class |
|---|---|---|---|
| Starter | each | $8 – $18 | Goods |
| Main course | each | $16 – $45 | Goods |
| Side dish | each | $5 – $12 | Goods |
| Dessert | each | $7 – $16 | Goods |
| Soft drink | each | $3 – $6 | Goods |
| Wine by the glass | each | $9 – $22 | Goods |
| Service charge — party of six or more | check | $10 – $120 | Goods |
The format this trade issues
The guest check: server, table and cover count in the header, modifiers indented under their parent dish, and a suggested-tip ladder under the total.
Get Receipt renders it as Restaurant guest check — Table-service guest check with a gratuity line and a customer signature rule under the total.
The full field order, alignment grid and physical spec for that format is documented on the restaurant guest check spec page.
Common questions
- What is the difference between the guest check and the card slip?
- The check is the itemised bill you bring to the table. The slip is the card terminal’s authorisation record, printed twice — the merchant copy carries the blank tip and total write-in lines, the customer copy carries the copy legend and the statement-verification sentence.
- Is an automatic gratuity taxed?
- A mandatory service charge is generally treated as part of the sale and taxed; a voluntary tip is not. That difference is why the two must never share a line.
- How should modifiers print?
- Indented two spaces under the dish they modify, priced or marked NO CHARGE. A modifier at the same indent as a dish reads as a separate item and gets queried.
Card details on this receipt
Get Receipt prints at most four card digits and has no expiry field anywhere in the app. FACTA forbids printing more than the last five digits, or the expiration date at all, on a cardholder receipt — so a violating receipt is not something this tool can be made to produce, by hand or by the assistant.
